Western Undergraduate Exchange/Texas Waiver 

Insurance and Medical Requirements 

1. Schedule a Physical Exam with your doctor.

2. Print and Have Your Doctor Complete the Physical form Provided by EAC.

3. Print and Complete the Insurance information form (requires parent or guardian signature if under the age of 18*).

4. Include a Copy (Front and Back) of Your Insurance Card.
